I used Skype yesterday between two far-away locations.
The audio quality was suprisingly good. It sounded like a regular telephone call.
Both parties had cable modem service, and sit behind consumer-brand routers (eg: Linksys).
One party had a wired ethernet, one had wireless (and could roam around while talking, looking for a comfortable place to sit).
One party had a boom microphone and used built-in laptop speakers. The other used a $1 junk microphone and built-in laptop speakers.
Both parties used Windows-XP.
One of the parties in the US routinely uses Skype to communicate to business partners in Moscow, having implemented it in the DMZ of a corporate network (after opening up the right ports).
Even though Skype is currently beta, I would recommend it to others. They say their plan is to continue offering free voice-ip, as they add pay services (eg: video, call-to-telephone-number, etc) in the future.
And you can’t beat the audio quality.
